PHOTOGRAPH PERMISSION

I hereby consent to my child's photograph being taken on behalf of Rudgwick Netball Club.
I understand that any such images may be used for promotional/publicity purposes (including Rudgwick Netball Club’s website), and that they will be used expressly for this purpose and will not be passed to a third party.

I also consent to video footage of my child being taken by Rudgwick Netball Club which will be used for training purposes only.
